What is CO2 Laser Resurfacing? (And Who Shouldn't Get It)

CO2 laser resurfacing is an ablative fractional laser treatment that removes microscopic columns of damaged skin to stimulate collagen. It treats fine lines, sun damage, acne scarring, and skin texture. Downtime is typically 5 to 7 days for standard fractional protocols and shorter for lower-energy settings. It's not suitable for active acne, recent Roaccutane use, pregnancy, or certain skin conditions.
How CO2 laser resurfacing works
CO2 laser emits a 10,600 nm wavelength that is strongly absorbed by water in the skin. A fractional CO2 laser — the type used in modern aesthetic clinics — delivers this energy in a grid of microscopic columns rather than ablating the whole surface.
Each column vaporises a tiny zone of damaged skin. The surrounding tissue is left intact, which means collagen and elastin remodel dramatically over 3-6 months, the skin heals faster than fully ablative CO2, downtime is days rather than weeks, and results continue to improve well after the surface has healed.
Fractional CO2 can also be delivered at lower energy settings for clients who can't take extended downtime — typically 1-3 days of pinkness rather than a week of peeling. Kelly tailors the energy level at consultation based on your skin and downtime tolerance.
What CO2 laser treats well
Fine lines and early wrinkles (especially around the eyes and mouth), sun damage and uneven tone on face, neck, decolletage, and hands, acne scars (especially rolling and boxcar scars), enlarged pores and uneven texture, pigmentation associated with photoageing, and surgical and injury scars.
Who should not have CO2 laser
Do not proceed with CO2 if any of the following apply: pregnancy or breastfeeding, Roaccutane (isotretinoin) in the last 6 months, active cold sores or bacterial skin infection in the treatment area, untreated active acne, recent sunburn or recent tanning (including self-tan), history of keloid or hypertrophic scarring, Fitzpatrick skin type V or VI (higher risk of post-inflammatory hyperpigmentation), or certain photosensitising medications.

Downtime and aftercare
Day 0 (treatment day): Redness similar to sunburn. Mild swelling possible. A gritty sandpaper feel as the micro-columns seal.
Days 1-3: Peak redness and swelling. Skin feels tight and rough. Lower-energy protocols usually resolve by Day 3.
Days 4-7: Standard CO2 protocols peel and flake. Do not pick — this is the single biggest cause of post-treatment pigmentation and scarring. Use only the gentle cleanser and occlusive balm your practitioner provides.
Weeks 2-4: Pinkness fades. Skin looks visibly clearer and smoother.
Months 2-6: Collagen remodelling continues. Texture and tone keep improving.
Ongoing: SPF 50+ every single day. This is non-negotiable.
How many CO2 treatments do you need?
Most clients see meaningful results from one well-dosed CO2 treatment, though acne scarring and deeper sun damage typically benefit from 2-3 sessions spaced 8-12 weeks apart. Lower-energy protocols can be delivered as courses of 3-4 sessions closer together when less downtime is preferred.
